Das Prostatakarzinom (PCa) ist derzeit in vielen Ländern der häufigste diagnostizierte maligne Tumor beim Mann und die zweithäufigste Krebstodesursache nach dem Bronchialkarzinom. Für die Bundesrepublik Deutschland existieren keine epidemiologisch gesicherten Daten für das PCa, obwohl seit den 1970-er Jahren eine Vorsorgeuntersuchung, die die Palpation der Prostata einschließt, mit großem finanziellem Aufwand gefördert wird. Aktuellere epidemiologische Daten liegen aus der Region München vor. Im Jahre 2001 war das PCa nach Angaben des Tumorregisters München hinter dem Lungenkarzinom die zweithäufigste malignombedingte Todesursache bei Männern. Das mittlere Lebensalter betrug bei Diagnosestellung des PCa 68 Jahre. [...]

Das PCa ist eine bezüglich der Histologie und anatomischen Variabilität heterogene Erkrankung. Aufgrund unterschiedlicher lymphogener Metastasierung und auch unterschiedlicher Wahrscheinlichkeit der Detektion mittels üblicher Prostatabiopsiestrategien ist die Lage eines PCa innerhalb des Drüsenkörpers ein prognostisch bedeutender, bisher unzureichend erfasster, Parameter für die Beurteilung der individuellen Risikoabschätzung. Die ultraschallgesteuerte Prostatabiopsie wird zurzeit in einer systematischen Sextantenbiopsie unter Berücksichtigung der basisnahen, mittleren und apexnahen Portion jeweils des rechten und des linken Prostataseitenlappens durchgeführt. Neuere Studien 5;6;9;16;19;19 [sic] ;29;34;36;38;45;55 zeigen die Notwendigkeit einer Modifikation dieses Vorgehens um die Quote falsch negativer Ergebnisse zu senken.
