Hodge zeigte als erste 1989, dass die systematische, ultraschallgesteuerte Prostatabiopsie eine höhere Karzinomdetektionsquote aufweist als die Biopsie suspekter Areale allein 34. Seither wird sie überwiegend als Sextantenbiopsie der Basis, der Mitte und des Apex des jeweils linken und rechten Prostataseitenlappens durchgeführt (Abbildung 10).

Eine signifikante Anzahl an Patienten (20-40%) mit erhöhtem PSA haben in einer Folgebiopsie einen positiven Befund, so dass man schlussfolgern kann, nicht alle Prostatakarzinome mit den regulären sechs Stanzbiopsien zu detektieren 36;41. Dieses Protokoll liefert selbst bei entsprechend exakter Indikationsstellung ein falsch negatives Ergebnis in 30% der Fälle 42. Norberg et al. zeigten, dass 15% der Karzinome durch die reguläre Sextantenbiopsie nicht entdeckt werden 45.

Mehrere Autoren fordern daher Variationen des Protokolls durch zusätzliche Biopsien in differierenden Anteilen der Drüse 5;6;9;16;19;22;36;38;45;50;55;57;58. Zunächst modifizierte Stamey dieses Schema, indem er die Entnahme aus dem lateralen Drittel der Prostata vorschlug, da so die periphere Zone als Hauptentstehungsort der Karzinome besser abgedeckt wird 58 (Abbildung 11).
